SPORTS PHYSICALS & CONCUSSION FORMS: Sports Physicals & Concussion Forms MUST BE TURNED IN TO THE HS OFFICE before sports practices on August 15, 2022. It is preferred that these forms are turned in by Friday, August 12, 2022. Athletes will not be allowed to practice (starting August 15) without a COMPLETED & SIGNED Sport Physical and Concussion form! FALL DUAL SPORT ATHLETES: Athletes who wish to dual sport must declare this intent by August 8, 2022. Please email: ssmith@usd111.org. Include the student’s name and the sports they plan to participate in.
