Dear Parent/Guardian,
Mosaic Sports Medicine has transitioned to using Sway Medical for baseline concussion testing to better evaluate and treat head injuries. This tool is widely utilized in professional, collegiate, and high school sports to manage concussions effectively.
Who Needs to Be Tested:
Student athletes in grades 7th, 9th, and 11th
All football players
Any new students(athletes)
Testing Schedule: We will conduct testing sessions on Wednesday, August 14. Please note the following session times:
Session 1: 8:30 AM
Session 2: 9:30 AM
Session 3: 10:30 AM (if needed)
How to Sign Up:
To sign up for a session, please use this survey link.
About the Test:
Duration: 20-25 minutes
Format: Non-invasive, “video-game” type format
Device: Student's phone or a provided device if necessary
Importance of the Test:
The baseline test helps in determining the severity of a concussion and monitors recovery, ensuring safe return-to-play decisions. Data from both preseason and post-injury tests will be evaluated by a concussion coordinator and physician.
Preparation:
Download the Sway app from the Apple or Google Play stores.
Ensure the app is installed on the student’s phone before testing.
Indication on the survey if an alternate device is needed.

The baseline test is a requirement for your son or daughter to participated in Doniphan West Athletics. The completed consent form must be turn in prior to the test being adminstrered.
